The Centering Healthcare site license number may be obtained by one of the methods below:

1) Contact your site's Centering Coordinator for the license number. 

2) Visit store.centeringhealthcare.org and log in to your account. 

• Click on "Account"

• Navigate to the "Account details" section 

• Look for "Site ID"

3) If you are unable to locate the site ID, please email the Centering Healthcare Client Relations team at help@centeringhealthcare.org and provide your: 

a) First and Last Name 

b) Site Name 

c) your role with the site
