The Centering Healthcare site license number may be obtained by one of the methods below:

1) Contact your site's Centering Coordinator for the license number. 

2) Login to the Centering Healthcare portal. The site license number will be located under the "Notices" section on the Dashboard page.

3) Email the Centering Healthcare Client Relations team at help@centeringhealthcare.org.  Please provide the name of the site in the email request.
